The ballot style handed to you when you vote is determined by your district/precinct.  To find out your precinct code:

Go to https://www.voterview.ar-nova.org/
Click the Search your Polling Place link
Select the Election
Select the County: Sebastian
Enter Last Name and Date of Birth
